INSURANCE & FEE POLICY
We are an in-network provider for Blue Cross Blue Shield, United Healthcare, and Medicare. You are responsible for any copayment or coinsurance at the beginning of your visit. If you have any questions about your coverage please contact your insurance company.
We are considered out-of-network for all other insurance plans; however, many of the health insurance policies partially or fully reimburse our services. You will be provided with a statement that includes all the information that insurance companies require to process claims. You would be responsible for the full fee at the time of service, and any reimbursement would be sent to you directly. It is suggested that you verify that your benefits include out-of-network reimbursement.
You also have the option to pay out-of-pocket, and not use insurance. Some benefits of self-payment may include less limits on the length and frequency of sessions.
Your treatment plan is designed to maximize continuous benefit for you. Missed appointments interfere with therapeutic momentum and can lessen the effect of therapy. However, we understand that, occasionally, circumstances arise that may require you to cancel an appointment. In these events, 24 hours notice is required so that your appointment time can be offered to another person. Except in the case of emergency, cancellations without 24 hours notice are charged at the full session fee (not your copay/coinsurance).
